Don’t use !!
I ended up having to do everything from scheduling the time, to call in New York State to get a copy of my card and number. I called the customer service number Roz, who said she was a supervisor was super rude and never helped me and said she would call me back but a month later I still haven’t heard from anyone. I would never recommend this company to anyone ever again.

To clarify for others reading, Elevate Holistics provides the medical evaluation and submits the electronic recommendation to the state, but the medical card itself is issued directly by the New York State program, not by our team. Once the recommendation is submitted, patients access their card through the state system.
Our records show that your recommendation was successfully completed and transmitted, and our team attempted follow-up outreach to assist when questions came up about accessing your state card. We also understand that there was some confusion during a supervisor call regarding how New York’s process works, which can understandably be frustrating.
We’re always happy to help walk patients through the next steps with the state if anything is unclear, and our support team remains available if you’d like additional assistance.
