I pay 76,000 THB per year for April International health insurance (2000 euros). Today was the first time I tried to use my outpatient benefits for a genuine doctor-ordered diagnostic blood test at Bangkok Hospital Phuket.
I arrived at 8:30 AM, the doctor examined me, identified clinical concerns, and ordered diagnostic tests. The hospital submitted the pre-authorisation request to April. Then the waiting began.
I called April's hotline three times. Each time I was told someone would "chase" the approval. Nothing happened. At 1:42 PM, over 5 hours later, April sent the hospital an email saying they were "unable to finalize" and recommended I pay a deposit out of my own pocket to "avoid any delay." Five hours of delay caused entirely by them, and their solution was for me to pay anyway.
I waited more. Called again. Their final answer? "We'll let you know tomorrow if we approve." Tomorrow. For a routine blood test. I had been sitting in the hospital for over 5 hours and their answer was to come back tomorrow and maybe they'll decide.
I left the hospital at around 2 PM with no blood drawn, no tests done and nothing to show for an entire wasted day except dehydration and frustration.
When they finally approved it, it was almost 4 PM, for tests that require fasting. So I now have to come back a second day for what should have been a simple outpatient visit.
But it gets worse. They cherry-picked what to cover from what the doctor ordered. A doctor at a hospital in their own recommended network examined me for fatigue, ordered tests, and April decided to reclassify some as "screening" and refuse others entirely. They linked a specimen storage request to hair loss even though my visit was for fatigue, which their own Letter of Guarantee confirms. Vitamin D, one of the most common causes of fatigue, they classified as a screening test rather than diagnostic. They are basically second-guessing qualified doctors to avoid paying.
When I emailed to dispute these decisions, they ignored my feedback entirely. The only replies I get are pre-generated templates telling me to use the Easy Claim app or submit forms.
I have been paying this company for years and barely used the policy. The one time I needed it for a straightforward outpatient visit, their "direct billing" system completely failed. This is not direct billing. This is a clown show. Direct billing means the hospital submits a claim, the insurer approves it in 15-30 minutes, and you walk out paying nothing. That's how every other insurer operates in Thailand. April's version of direct billing is making you wait until you either give up or pay out of pocket, which defeats the entire purpose of having insurance.
They advertise "Insurance Made Easy" in their logo. There is nothing easy about this.
I will not be renewing. If you are considering April International, understand that "direct billing" means you sit in a hospital for half a day while they decide whether to honour the policy you already paid for, then refuse to cover basic tests ordered by a doctor in their own network, then ignore you when you complain. Save yourself the trouble and just pay out of pocket. It's cheaper and faster.

Très mauvaise expérience. Actuellement étudiant à l'étranger depuis 2 mois, j'ai été contraint de souscrire à une assurance à l'étranger pour réaliser mes études.
April international à ce jour ne m'a jamais remboursé malgré plusieurs demandes qui ne dépasse même pas 30 euros, sachant que l'entreprise génère plus de 800 millions d'euros pour l'année 2024. Malgré l'envoie de l'intégralité des documents nécessaires, les demandes de remboursement sont contestées. Lorsque vous transmettez les documents demandés après la première demande pour accomplir le remboursement, on vous demande de nouveaux justificatifs qui n'ont parfois aucun lien avec la demande initiale dans le seul but d'épuiser le demandeur et d'éviter de vous rembourser. (par soucis de rapidité, April demande des justificatifs au compte goute, pas mal non ?). Certaines demandes sont refusées immédiatement sans avoir l'opportunité d'envoyer les justificatifs, alors même que le relevé de non-remboursement exige la transmission de nouveaux documents pour obtenir remboursement. Donc April vous empêche de transmettre les documents malgré le fait qu'on vous les demande, c'est vraiment bien n'est ce pas ? Tout cela vous oblige donc à reproduire une nouvelle demande de remboursement plusieurs semaines après celle que vous avez initié à la base.
Je déconseille le choix de cette assurance qui est dans une logique purement marchande et complètement désintéressé de sa mission d'assureur. Pour toute personne qui souscrit par malheur à cette assurance, il ne faut surtout pas dépendre de cet organisme pour votre présence à l'étranger.

Refus catégorique de remboursement.. même partiel.
Voilà une mutuelle rayée de ma CARTE ! !!
Il y a vraiment mieux ! !!

I contacted APRIL International while comparing and setting up my health insurance in Thailand, and Nathan was my main point of contact.
He consistently provided clear, structured, and prompt answers, often replying early in the morning (around 9 am sharp!), even when I asked detailed questions about coverage, exclusions, and policy options (hospitalisation, dental, medical history declarations, etc.).
He shared relevant links and documentation, which made the whole process much easier to understand and complete. The explanations were precise and helped me make informed decisions.
In terms of pricing, the plan offers solid value, with coverage up to 10M THB per year for around 35,000 THB annually, which is competitive for this level of coverage in Thailand.
I have not used the insurance yet, so I cannot comment on the claims process. However, based on the onboarding experience and support received, I would recommend APRIL International.
I highly recommend April :)

Mauvaise expérience suite à un séjour de 6 mois à l'étranger. Urgence dentaire non prise en charge car le "texte" sur la facture n'était pas assez clair. On ne va pas chez le dentiste par plaisir !
Je vous conseille donc de choisir une autre assurance moins à la recherche du profit avant tout.
